What is Sonodynamic Therapy?

Sonodynamic therapy is a new cancer modality, which has huge potential in the treatment of cancer. Compared to photodynamic therapy (PDT), sonodynamic therapy has the advantage that ultrasound can penetrate deeply into the body. As you can see, we use low-intensity ultrasound to activate sensitizers in combination with ozone therapy or a hyperbaric oxygen chamber to increase oxygenation in cancer tissue.

Researchers collaborating with Saisei Mirai on SDT & PDT

Saisei Mirai has been collaborating with researchers in different universities on sonodynamic therapy and GcMAF for many years (including Tokushima University, Tottori University, and Asahikawa University of Medicine).

This experiment was conducted by Dr. Shibaguchi of Fukuoka University. Picture A and picture B both show ultrasound application to mice.

You can see the changes in tumor size and tumor volume before SDT and after SDT.

Sonodynamic therapy can be taken in a hot bath tub to target many cancer tissues in the body.

Disseminated Tumor Cells (DTCs) and Circulating Tumor Cells (CTCs) - For a long time, bone marrow and the blood stream were suggested as cancer remaining places. Complete remission doesn’t mean there are no cancer cells in the blood or in the bone marrow.

Sonodynamic therapy has been considered as a local treatment to destroy local cancer tissue. But we may change this concept, because sonodynamic therapy can be taken in a hot bath tub as a whole body treatment. It can target primary tumors, both known and unknown, metastatic tumors, both known and unknown, circulating tumor cells in the blood and disseminated tumor cells in the bone marrow.

Sonodynamic therapy (SDT) photosensitizers and sonosensitizers are basically non-toxic and are preferentially taken up by cancer cells. The sensitizers are normally taken sublingually 24 hours prior to ultrasound application, but recently, we developed a new sonosensitizer which is taken by intravenous (IV) injection 1 hour prior to ultrasound application. Energy causes the activation of sonosensitizers, producing singlet oxygen and free-radical oxygen in cancer tissue, leading to coagulative necrosis. We usually combine SDT with GcMAF, which brings patients maximum treatment effects. 

What is Ultrasound cavitation?
Ultrasound is physical energy that causes ultrasound cavitation, that is, the formation of microbubbles when pressure is relatively low. This usually occurs when a liquid undergoes rapid changes in pressure—alternating between high and low pressure. As the microbubbles increase in size and finally explode, they generate shock waves.

When microbubbles collapse in the microscopic world, they generate shock waves, high temperature of more than 5000 degrees Celsius, High pressure of more than 1000 atmospheric pressure and Strong light. When microbubbles don’t collapse, they still generate ultrasonic microstreaming caused by vibrational size changes of microbubbles.

In H&E staining, tumors treated with ultrasound (US) and SDT showed ruptured blood vessels surrounded by pyknotic nuclei 24 hours after treatment. Necrotic cells were observed most commonly in tumors treated with SDT. Nuclei of tumor cells and tumor blood vessels were intact in the control and TONS504-Mn groups.
Histological changes after the treatment. In Hematoxylin-eosin staining, tumors treated with ultrasound and SDT showed ruptured blood vessels surrounded by pyknotic nuclei 24 hours after treatment. Necrotic cells were observed most commonly in tumors treated with SDT.

In Ki67 staining, Ki67-positive cells in the SDT group showed a significant decrease in number compared with other groups.
